Your Opportunity:

The RN Interventionist position, with the Adult Team in Home Care, is responsible for assessing, planning, organizing, implementing, reviewing and evaluating the needs of shorter-term clients (Acute and Rehabilitation clients) and their caregivers/families requiring community-based services. The RN Interventionist fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involving provision of services and may work in multiple settings. The RN Interventionist is also responsible for providing direct nursing interventions to a select group of Long Term Supportive, Maintenance and End-of-life clients.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): https://connect.nurses.ab.ca/home/learning-and-development/learning-modules NextGenU Health Assessment Resources Certification: https://courses.nextgenu.org/course/view.php?id=271.

Additional Required Qualifications:

1 to 2 years of recent Home Care experience or an equivalent combination of education and experience. Excellent interpersonal and communication skills. Excellent organizational and time management skills. Demonstrated critical thinking and decision making skills. Demonstrated assessment skills. Demonstrated ability to operate within an electronic health record. Proven ability to work in a multidisciplinary team. Ability to work independently using innovative approaches to case manage a diverse client caseload. Good negotiation and conflict resolution skills. You will be responsible to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. Basic computer skills required. Knowledge of ethical principles and their application in the clinical/home setting. Ability to function well in a fast paced, challenging environment. Physically able to perform all assigned tasks. Ability to perform all duties in accordance with related legislation. This position requires a vehicle for business use. Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel is required.

Preferred Qualifications:

Experience with mental health and/or addictions/harm reduction. Experience working in community based setting.
