As the Registered Nurse Case Manager covering County of Flagstaff, you will have the opportunity to make a difference in the lives of people in your community. Reporting to the Care Manager, the Case Manager is responsible for directing the care of clients under the home care program. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning and facilitate client and family teaching. Your experience in providing wound care and knowledge of the wound care principles and products, as well as palliative and home parenteral therapy will be essential to making you successful in this position. Located in east central Alberta, the County of Flagstaff offers beautiful big open spaces, salt of the earth resilient folks and the opportunity to be part of a vibrant community. There’s nothing as beautiful as the dance of sun-kissed canola in a breeze, or a prairie thunderstorm’s electrified sky. Farmer’s Markets, small town rodeos, artisan beer brewing and summer festivals are the perfect way to spend your downtime. In oil and gas, agriculture, and land-based recreation activity lies the ingredients for a life authentically lived in Flagstaff County.
