Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

The Registered Nurse - Triage position is a combination of two roles within the Home Living Program (Triage RN & RN Case Manager). As a Triage RN, you will require strong assessment, communication and interpersonal skills in order to assess and assign the incoming flow of referrals into the network office. As an RN Case Manager, you will play a key role in linking the needs of Home Care clients and service delivery. Using your strong assessment, organizational and leadership skills, you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required (service options, agencies, acute and continuing care facilities, etc.). Through the development of the Care Plan, in collaboration with the client's family, you will ensure services are provided and modified on an ongoing basis, based on the client's response to the intervention. Experience in the Home Living Program is preferred. This position will be based out of the Morinville office, but may provide support to home care's Suburban/ Rural West team, which may include travel to the other home care network offices, as required.

Preferred Qualifications:

Previous Community Care and/or Home Care experience preferred. • Experience with Meditech and the RAI Home Care assessment tool is preferred.
