Registered Nurse - Certified Diabetes Educator

Registered Nurse Certified Diabetes Educator - Diabetes Specialty Clinic - Medical Specialties Clinic Red Deer Regional Hospital: The RN CDE is an important part of an interdisciplinary team that ensures appropriate diabetes care in an outpatient setting. The RN CDE will provide care management of Central Zone Diabetes Specialty Clinic patients and are responsible for assessment, development, implementation, and evaluation of client/patient care plans in chronic disease management. Provides expertise to internal and external professionals as needed. Uses counselling, behavioral change theories/strategies as appropriate to support clients working towards their desired health outcomes. This position can be hybrid (on and off site). Being on call occasionally for insulin pump starts is required. The RN CDE will: Provide individual education sessions to clients and families with diabetes. Assess client knowledge and understanding of diabetes and adapt education plans to meet needs. Teach administration of insulin, including titration and management per insulin adjustment protocols. Teach self monitoring of blood glucose. Collaborate with the health care team and client to develop and implement diabetes management plans. Assist in creating individual plans to incorporate nutrition, activity and medication. Employ information technology (Blood Glucose Meter, Continuous Glucose Monitoring, Insulin Pump Therapy). Monitor and adjust treatment plans to support clients in achieving optimal blood glucose control. Offer support to clients dealing with the challenges associated with diabetes and connect with relevant supports. Monitor client blood glucose records and other relevant data in regular follow up visits. Manage readiness for Insulin pump therapy and manage clients on insulin pumps. Stay up to date with the latest research and developments in diabetes management.

You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:Certified Diabetes Educator with experience working in an Ambulatory Care Setting. Must have excellent communication and documentation skills and the ability to participate as an interdisciplinary team member. Must also be able to work independently with high organizational and prioritization skills.
Preferred Qualifications:Preferred experience in the Central Zone Diabetes Clinic. Experience working with insulin pumps. Experience with providing patient education.
