Your Opportunity:

As the Community Care Case Coordinator with Castor Community Care, you will act as a close liaison with family members, significant others, physicians and health care providers in acute, long-term and community care, while assessing, planning, implementing, and evaluating care for clients using Nursing Diagnosis. Castor, a picturesque, lively community is located approximately 143 km east of the city of Red Deer. It was first developed to meet the needs of growing homesteaders and agriculture has continued to be an industry mainstay, along with a growing oil and gas industry. In the 1920 Castor, residents began a tree-planting program which helped lead to the beautiful tree lined streets you see today. Castor provides a wide assortment of recreational activities including golfing, swimming, camping, skating, skiing and more. Note: This position requires a vehicle for business use. Possession of a valid driver's license and access to a motor vehicle in good repair for work-related travel is required.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Must be able to meet the physical demands of the job. Good communication and observational skills is required. Reliable and responsible, to provide quality, and consistent care to home care clients. Good routine practices regarding infection control and personal care.

Preferred Qualifications:

Familiarity with using a PC, Internet, and Microsoft Office applications is an asset.
