Your Opportunity:

As a member of Specialized Geriatric Services (SGS), the Geriatric Outreach Registered Nurse provides comprehensive, patient-centered care within the community, while also supporting the Zone Seniors Ambulatory Clinic and Acute Care of the Elderly (ACE). The Outreach Nurse assesses, consults, and coordinates care for frail seniors living in their homes and community settings. Responsibilities include conducting home visits, completing cognitive and functional assessments, administering standardized tools such as the SLUMS examination, and supporting medication coverage applications (e.g., Aricept through Blue Cross). Working collaboratively with SGS physicians and the interdisciplinary team, the Outreach Nurse completes Comprehensive Geriatric Assessments (CGAs) and develops individualized care plans for older adults with complex health needs. Care plans are created in partnership with clients and/or their agents and shared with healthcare providers and community partners to support coordinated care. Ongoing follow-up is provided to evaluate outcomes and adjust care plans as needed. The role also includes clinic support, including coverage for the Camrose clinic during staff absences and participation in the Red Deer clinic, where assessments and documentation are completed following client visits. The Outreach Nurse participates in interdisciplinary team meetings, contributes to care planning, and provides education and support to clients, families, and care providers to promote optimal health outcomes. Significant travel is required throughout the service area, including Dayton Valley to Consort and Vermilion to Stettler, with driving being an essential component of the position.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:

Excellent oral and written communication skills and proficiency in computer skills. Demonstrate strong critical thinking and problem-solving ability in a team environment. Demonstrated ability to function and collaborate within an interdisciplinary team focused on quality client outcome through best practice consultative care recommendations. Excellent organizational skills, and time management skills are required. A working knowledge of community resources within the Camrose and surrounding area is required. A minimum of five years’ experience working in Continuing Care - Homecare and gerontology within the past seven years. Additional training in Gerontology is required with knowledge, skills and experience performing functional and cognitive screening tests such as the Geriatric Depression Scale (GDS), Geriatric Anxiety Inventory (GAI), Frontal Assessment Battery (FAB), Functional Activities Questionnaire (FAQ), Addenbrooke Cognitive Examination III (ACE III), Trails A & B.

Preferred Qualifications:

Master level degree in nursing is preferred. CNA certification in Gerontology is preferred. Previous experience with a Geriatric Outreach team is preferred. Experience conducting an environmental risk assessment and safety visit plan is preferred. Experience working with patients with a major neurocognitive disorder and/or cognitive impairment in a community setting is preferred.
