Your Opportunity:
Transition Services Coordinator Inpatient and Emergency Department, screens, assesses, coordinates and evaluates the needs of patients and families who cannot function independently in the community. This position entails coordinating a broad range of services and community resources. This includes applying case management principles as they relate to the transition of clients through the continuum of care and requires critical thinking, organizational skills, attention to detail, and accuracy of work. This may include arranging home care support and linking patients to the appropriate community agencies. Provide clients/families with information and counseling with respect to care options and facilitates access to these. The TSC establishes and maintains effective communication and liaise with physicians and others involved in the client’s care. Depending on hospital capacity, work must be completed within fixed deadlines, coping with frequent interruptions and care changes. The RN must exhibit excellent interpersonal communication, conflict resolution, organizational, critical thinking, and assessment skills. The successful candidate will be expected to cover and understand all caseloads and TS processes.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:1 year (in the last 3 years) worked community experience (TS/homecare/ISFL). Physically able to walk 3-5 km daily; carry/lift/pull resource materials weighing up to 25 lbs; be on feet long periods; provide physical assistance to clients if intervention for safety required; manage repetitive adjustment and exertion, including sitting/standing/bending. Ability to work with a diverse client population, including clients with complex medical, mental health, and behavioral care needs, challenging behaviors, social situations/dynamics. Ability to work in a fast-paced environment and manage stressful situations. Excellent organizational and time management skills to efficiently organize and prioritize workload. Demonstrated ability to provide coverage in all TS caseloads, flexibility with assignment/caseload changes as indicated by operational needs. Perform up to 6 hours up keyboarding (min 40 wpm) and written documentation per shift while navigating a variety of IT programs.
Preferred Qualifications:Minimum 2 years medical/surgical nursing experience required. Bachelor's in nursing preferred, however, skills and experience relevant to nursing practice in community sector considered. Able to understand and apply case management principles as it relates to the transition of clients through the continuum of care. Must have excellent communication, conflict resolution, and organizational skills to work with the multi-disciplinary team.
