Your Opportunity:

Reporting to the Area Manager, Red Deer Seniors Health, in the role of Registered Nurse - Home Care Case Manager, Community Response, you will be part of the Intensive Home Care team and the Community Response team. These teams are responsible for coordinating and providing quality care to clients in the community that are high risk for being hospitalized and to help facilitate seamless discharges from hospital. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning. Through the development of the care plan you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): https://connect.nurses.ab.ca/home/learning-and-development/learning-modules NextGenU Health Assessment Resources Certification: https://courses.nextgenu.org/course/view.php?id=271.

Additional Required Qualifications:

Minimum 3 years community (Home Care) experience required. Strong knowledge of home care program, the acute care environment, community resources, and stakeholders is required. Demonstrated excellent communication skills in complex and dynamic environments. Ability to work independently as well as being a contributing member of the team.

Preferred Qualifications:

Knowledge of, and comfort with, Connect Care, RAI-HC, and other electronic documentation tools an asset. Ability to provide case management support is an asset. Experience with HPT, wound care, client teaching are assets. Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Current Intensive Home Care experience is preferred.
