Your Opportunity:

This position is based out of the South Complex at Red Deer Regional Hospital Centre (RDRHC). This position is a combination of two distinct roles that support inpatient rehabilitation services in Central Zone. Three days a week the successful candidate will work with a team of social workers, registered nurses and an administrative assistant to process referrals and match patients to the four inpatient rehabilitation programs in the zone; 1) Unit 35 at RDRHC, 2) SAGE in Two Hills, 3) DOAR in Daysland and Restorative Care in Lacombe. The Central Intake team work closely with all acute care sites in Central Zone in order to process referrals quickly and effectively. They have strong working relationships with all four rehabilitation programs and collaborate to provide patients with an exceptional rehabilitation journey. Two days per week the successful candidate will work as Transition Coordinator (TC) to support patient flow and discharge planning on Unit 35 at RDRHC. The TC will work closely with the Unit Manager, Assistant Head Nurse, and the Allied Health team to plan and coordinate discharges from unit 35 to home and other alternate locations after the completion of an intensive rehabilitation program. The TC will collaborate with internal and external partners, RDRHC Units, community resources such as home care and alternate living sites.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Preferred Qualifications:

Experience with triaging referrals and service-matching patients to programs preferred. Experience working with stroke rehabilitation within 3 of the last 5 years preferred. Experience with discharge planning and service coordination for patients preferred. Completion of the required EPIC courses for the transition coordinator role would be considered an asset.
