As the Registered Nurse Case Manager covering County of Flagstaff, you will have the opportunity to make a difference in the lives of people in your community. Reporting to the Care Manager, the Case Manager is responsible for directing the care of clients under the Home Care program. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning and facilitate client and family teaching. Through the development of the care plan, you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to relevant resources and/or provide direct care as needed. Your experience in providing wound care and knowledge of the wound care principles and products, as well as palliative and home parenteral therapy will be essential to making you successful in this position. You may be required to cover other assignments as workload dictates and be on call. As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:Valid Class 5 (non-GDL) driver’s license. Required to report to work within 60 minutes for on call duty. Must be able to meet the physical demands of the position: This position requires frequent physical demands including walking, bending, lifting, and standing and driving for extended periods of time. Transferring patients of varying sizes and degrees of mobility. Transporting supplies and equipment. Driving in all weather conditions. You may be eligible for the Rural Federal Student Loan Forgiveness Program if you meet other eligibility requirements.
Preferred Qualifications:Appropriate competency in Meditech, Care Manager and RAI-HC programs and knowledge of Outlook and MS Teams. Broad knowledge of provincial, regional, and local community resources. Minimum 1 year experience within the last 5 years in a Home Care environment.
