Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Three years recent experience in Community Health setting as a Case Manager (within the last five years) is preferred. Excellent interpersonal, leadership, conflict resolution and communication skills. Excellent time management skills. Excellent critical thinking and decision-making skills. Ability to be self-motivated and work independently is required. Ability to monitor financial accountabilities related to care delivery. Physically able to perform all assigned tasks. Basic computer skills required. Daily Physical Exertion & Tasks can be, but not limited to: Standing, walking, lifting, pulling, pushing, stooping and carrying during the entire shift up to 50 Lbs (~ 23kg)

e-Documentation training. Home Care experience.
