Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:A minimum of 3-5 years in Integrated Home Care program (transition, home care, and palliative) OR 3-5 years nursing practice with a demonstrated knowledge of community resources. Demonstrated critical thinking, organizational skills, attention to detail, and accuracy are required. Ability to communicate and collaborate with the multidisciplinary team is required. Ability to work in a fast paced department that is variable, unpredictable, and has competing priorities is required. Ability to work with fixed timelines, cope with frequent interruptions, and changes required. Demonstrated understanding of case management principles as it relates to the transition of clients through the continuum of care. Ability to perform all assigned tasks and direct clinical care including but not limited to: Constant adjustment of position (sit/stand) throughout the shift. Physical exertion including bending/walking/sitting or standing for long periods of time. Keyboarding/moussing documentation for long periods of time.
Preferred Qualifications:Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in the community sector will be considered. Meditech experience (Care Manager, PCS) preferred. Strong conflict resolution and change management skills are preferred. Additional educational, advanced skills, and clinical preparation across the Senior Health spectrum (Home Care, Long Term Care, Access Center, Palliative Care and Specialty Geriatric Areas) strongly preferred.
