Bachelor's degree in related field. Well versed in the Continuing Care health service standards. Strong understanding of Continuing Care (Long Term Care, Supportive Living, Home Care).
Additional Required Qualifications:
5-7 years in Continuing Care environment with broad knowledge of the Continuing Care system. Experience facilitating improvement activities/initiatives and knowledge or ability to scale/spread. Proficiency in MS Office - Excel, PowerPoint, Word, Outlook. Ability to work with limited supervision, high degree of independence. Strong/exceptional communication skills; both verbal and written. Exceptional organizational skills and ability to prioritize multiple competing demands. Experience in educational plan development and delivery of content, working with adult learners. Leadership skills including demonstrated comfort with decision making and collaboration to inspire and influence change. Advanced skillset in data and evaluation.
Preferred Qualifications:
Experience in Health Infomatics, Quality Improvement, Standards, Auditing. Understanding analytics. Case Management experience. Understanding of the resident assessment tools (RAI).
