Masters’ degree in health related discipline or equivalent experience. Experience in home care and community care sectors of continuing care. Quality Improvement certification (Green Belt). Change Management certification (PROSCI or equivalent).
Additional Required Qualifications:
Minimum 5 years of quality/practice improvement related experience in a healthcare environment. Strong understanding of interRAI suite of instruments (MDS 2.0, RAI HC, etc.). Proven knowledge of clinical informatics and/or experience with Connect Care project. Expertise in MS Office suite, including MS Project and Visio. Clinical experience and/or broad knowledge of Home Living, Supportive Living and Long-term Care processes. Demonstrated skills in mentorship, leadership, application of quality improvement, knowledge translation processes, provider education and curriculum development. Proven ability to foster relationships and partnerships with highly complex internal and external stakeholders. Demonstrated ability to work both collaboratively and independently within a large organization with a variety of staffing groups. Excellent interpersonal, communication, time management and leadership skills. Demonstrated abilities in conflict resolution and management. Ability to effectively prioritize and execute tasks in a high-pressure environment while conforming to shifting priorities, demands and timelines through analytical and problem solving capabilities. A demonstrated clear pattern of professional and personal development.
Preferred Qualifications:
Project Management certification. Lean Six Sigma or equivalent Black Belt. Gerontology Certificate. Leadership Certification. Professional Clinical Designation. Qualified interRAI Assessor (RAI-HC preferred). Current knowledge of the continuum of health services. Systems thinking approach. Creative, flexible, well-organized. Critical thinking and judgment skills. Proven facilitation skills.
