Required Qualifications:

Minimum Bachelor’s Degree in Nursing, plus appropriate licenses, and designations. Minimum of 3-5 years’ experience in AHS home care program. Experience progression from frontline and clinical experience to leadership roles involving strategy or policy development, education, service design and/or implementation. A superior knowledge of home care clinical operations, best practice, and governance to support practice. A demonstrated understanding of the complexity of the continuing care system and the different levels of care within Continuing Care. Demonstrated ability to address complex health issues and manage change in service delivery toward desired outcomes. Report development skills with demonstrated ability to translate data into information. Demonstrated pattern of professional and personal development. Equivalencies of education and experience may be considered.


Additional Required Qualifications:

The incumbent will possess a clear understanding of home care philosophy including personhood, community, and case management principles. Education or demonstrated ability in contemporary change management and quality improvement practices and methodologies. Experience with participation in the development and implementation of new programs or practices and/or in recommendation of new strategies and solutions. Proven ability to work with teams made up of internal and external stakeholders to achieve organizational goals. Proven understanding of project oversight, meeting deadlines and delivering required outcomes. Beginning ability to use strategic thinking, considering long-term goals, current state, and assessing options and implications. Effective communicator with strong verbal and writing skills. Leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching, teaching, and the ability to inspire and build confidence in others. Ability to be resourceful, creative, adaptive and flexible. Change management experience. Highly developed leadership skills including coaching, mentoring, solution finding. Ability to champion change across all delivery continuums including community, hospital, physicians, government etc. Respected voice of Home Care, with connections in all zones. Intermediate skills in Data analysis and interpretation.


Preferred Qualifications:

Master's Degree in a related professional discipline preferred. Skills in report writing and comprehensive presentation skills. Application of expertise on a provincial level. Understanding of the strategic direction for continuing care policies and services and consolidation of the strategic direction with results of the Provincial Senior's Health & Continuing Care projects to date. Experience working independently in an organization of significant size, complexity and diversity.
