Description:
The Practice Consultant serves as a trusted advisor to senior leaders, physicians, and inter-professional teams, empowering them with critical practice knowledge that enhances patient care across multiple streams. This role is responsible for supporting the development and implementation of provincial programs, shaping comprehensive evaluation frameworks, and collaborating with operational leaders to strategically enhance service delivery. With a provincial scope, the Practice Consultant provides expert guidance on transitions in care from acute settings to a range of community-based environments. The Practice Consultant influences system capacity and service delivery by transferring discipline-specific and system-level practice knowledge to healthcare professionals, Provincial Health Agencies, and care Operators across Alberta. This role also contributes to strategic planning and initiatives that improve service delivery, while managing project resources to ensure alignment with organizational goals.

A minimum of a Bachelor’s Degree in a related professional discipline, with a preference for Nursing, along with the appropriate professional licenses and designations. A minimum of five years’ experience is required in areas such as case management, system navigation, and transition services or discharge planning. A strong understanding of the structure and interconnection of health services and systems is essential, as is the ability to collaborate with stakeholders and disciplines across the province to develop and operationalize effective, scalable solutions.
Additional Required Qualifications:
The ideal candidate must possess Prosci / Change Management Certification and LEADS certification. Must demonstrate strong skills in report development, including the ability to translate data into meaningful information to support decision-making. Proficiency in Microsoft Office 365 and SharePoint is essential. As well a Yellow Belt or equivalent process improvement designation.
Preferred Qualifications:
A Master’s Degree in a related professional discipline is preferred, with Nursing being highly desirable. A Project Management Certification or an equivalent credential would be beneficial. Additionally, 3 to 5 years of progressive experience, ideally in healthcare operations, human services, leadership, strategy or policy development, service design, and/or implementation would be advantageous. A Green Belt or an equivalent process improvement designation would be valuable.
