Three to five years’ experience with Senior’s Health. Home care, dementia care, discharge planning, and experience with placement or working in designated living option environment. Strong leadership abilities with negotiation and conflict resolution skills. Demonstrated ability to navigate the healthcare system and advocate for clients/families.

Preferred Qualifications:
Management experience with ability to provide direction, team leadership, goal planning, and evaluation. Knowledge of computer applications (Netcare, Meditech, MS Office); demonstrated competency with RAI-HC; AHS McMaster Case Management; Supportive Pathways training and knowledge of cognitive status and functioning; experience with clinical assessment; knowledge of applicable legislation, and programs and community resources.
