The Northern Alberta Cardiac Rehabilitation Program is an exercise and education program geared towards providing participants with the knowledge, skills, and supports they need to better manage their health after a heart event. The Physiotherapist will play an important role in supporting patients through their cardiac rehabilitation experience. Working to achieve the vision of the Northern Alberta Cardiac Rehabilitation Program, care is focused on promoting exercise and healthy lifestyle management. Responsibilities of the Physiotherapist include: Teaching group education sessions about self-management and cardiovascular health incorporating principles of adult education. Facilitating group exercise sessions in collaboration with exercise specialists. Providing exercises assessment, training, rehabilitation, risk factor identification and self-management services. Providing 1:1 Physiotherapy consultation based on patient needs. Supporting cardiovascular functional assessments. Performing functional assessments based on a patients physical needs to better support their cardiovascular health. Working alongside a multidisciplinary team to manage a patients progress through their rehabilitation program. Maintaining current knowledge of evidence-based practice in exercise physiology as it relates to cardiovascular health and exercise guidelines. Understanding chronic disease and behavioural psychology. Motivational interviewing to support patients in make lifelong healthy choices. Participating in committees and quality improvement initiatives. Supporting cardiac rehabilitation research through influencing best practice.

Description:

As a Physiotherapist II, you will provide coordination of physiotherapy and other allied health clinical services. You will provide supervision of students and team members as assigned. You will work both independently and as part of an interdisciplinary team providing leadership to other healthcare professionals. You will be making day-to-day clinical decisions on patient/family needs regarding assessment, intervention, discharge and follow-up. You will make decisions and advise team members on complex clinical cases and provide clinical leadership within the program or service area to ensure safe, efficient and effective client care. You will ensure physiotherapy practice is evidence based and in alignment with Alberta legislation and regulations, professional standards of practice, code of ethics and AHS policy and guidelines. You will be coordinating the day-to-day operations of an assigned service area and monitoring care delivery, identifying service needs and assisting other medical staff with caseload management, completing chart audits and providing follow-up and feedback to staff. You will participate in program development and operational planning and representing the department on committees or programs. You will make decisions on staffing level requirements, workflow, caseload priorities in collaboration with managers and other healthcare providers. All AHS Physiotherapists have access to a list of experienced clinicians across the province and are able to tap into this resource via email, phone and virtual meetings. As we transition to our Provincial Electronic Medical Record (Connect Care) you will also be able to share patient information real time with these senior clinicians to assist you in providing high quality care.
