Physiotherapist II, Rehabilitation Navigator - Adult Specialized Outpatient Program. Please note, this position is open to Physiotherapists, Occupational Therapists, Recreation Therapists, Social Workers, Speech Pathologists, and Psychologists. The Glenrose Rehabilitation Hospital (GRH) is the largest free-standing, comprehensive tertiary rehabilitation hospital in Canada, serving patients of all ages who require complex rehabilitation to enable them to participate in life to the fullest. This is a collaborative program offering assessment and treatment from a multidisciplinary team of Allied Health Professionals. The Rehabilitation Navigator will coordinate and facilitate patient rehabilitation with a team of allied health clinicians and ensure the overall management of the level and quality of services received by patients is in accordance with best practice standards. Duties will include ensuring a smooth transition to outpatient services and from outpatients service to community based programs, efficient and timely processing of referrals and requests, the effective management of care based on establish performance and outcome indicators; and the optimal and integrated use of outpatient inter professional and community resources, in order to provide quality service for patients and their families in accordance with their goals and based on functional outcomes. Description:

As a Physiotherapist II, you will provide coordination of physiotherapy and other allied health clinical services. You will provide supervision of students and team members as assigned. You will work both independently and as part of an interdisciplinary team providing leadership to other healthcare professionals. You will be making day-to-day clinical decisions on patient/family needs regarding assessment, intervention, discharge and follow-up. You will make decisions and advise team members on complex clinical cases and provide clinical leadership within the program or service area to ensure safe, efficient and effective client care. You will ensure physiotherapy practice is evidence based and in alignment with Alberta legislation and regulations, professional standards of practice, code of ethics and AHS policy and guidelines. You will be coordinating the day-to-day operations of an assigned service area and monitoring care delivery, identifying service needs and assisting other medical staff with caseload management, completing chart audits and providing follow-up and feedback to staff. You will participate in program development and operational planning and representing the department on committees or programs. You will make decisions on staffing level requirements, workflow, caseload priorities in collaboration with managers and other healthcare providers. AHS is dedicated to supporting our Physiotherapists in their practice area and expanding their skillsets regardless of location. All AHS Physiotherapists have access to a list of experienced clinicians across the province and are able to tap into this resource via email, phone and virtual meetings. As we transition to our Provincial Electronic Medical Record (Connect Care) you will also be able to share patient information real time with these senior clinicians to assist you in providing high quality care.

Required Qualifications:

Completion of master's degree in Physiotherapy from an accredited university program or bachelor's degree in Physiotherapy from an accredited university program and related experience. Active or eligible registration and practice permit with Physiotherapy Alberta College + Association.

Additional Required Qualifications:

Completion of master's/bachelors degree in Physiotherapy, Occupational Therapy, Recreation Therapy, Social Work, Speech Pathologists, and Psychology from an accredited university program from an accredited university program and related experience. Active or eligible registration and practice permit with the applicable Alberta College + Association, governing body. Minimum of 3 years (current within the last 5 years) experience working with adults and or older adults with rehabilitation needs. Minimum of two (2) years’ experience in case management and service evaluation activities. Must have a solid understanding of population specific disability causation, risk factor management, medical complications and rehabilitation concepts. Must have: exceptional interpersonal communication skills, excellent written and oral skills, computer proficiency and have experience working independently and within a team. Must be able to sit, manage phone and virtual technology, use computer for long periods. Must be able to mobilize throughout a large site.

Preferred Qualifications:

Specific knowledge of the Glenrose services and relevant community services is an asset and connect care training is preferred.
