Your Opportunity:

As the Physiotherapist I - Home Care in this position you will be an integral member of the interdisciplinary home care team providing PT services to clients in their homes. Half of the position is based in Tofield serving home care clients in Tofield and the surrounding area; the other half of the position is based in Vegreville providing PT services to home care clients in the Vegreville area.

Description:

As a Physiotherapist I, you will be working both independently and as part of an interdisciplinary team providing a range of physiotherapy services to patients and families within the established standards, directives and policies for the work environment and the profession. These services include: assessment/screening, development of customized goals and treatment plans for therapeutic interventions, delivery of therapeutic interventions, education, discharge planning and follow-up to enable patients to achieve optimum function and independence. As a Physiotherapist I, you will include patients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will be an integral member of our team through provision of guidance to other healthcare professionals and clinical direction to therapy assistants, support personnel and students. You will be reviewing clinical referrals, screening and assessing client needs from a physiotherapy perspective in collaboration with other health disciplines as appropriate. You will be developing, planning and implementing physiotherapy services in cooperation with other disciplines in the hospital and community, and establishing and maintaining effective communication with colleagues, medical staff, and health care practitioners concerning treatment plans, progress and discharge. You will be participating in client, family and caregiver education in collaboration with other disciplines, producing and maintaining information, documentation and verbal reports to support the delivery of physiotherapy services. You will maintain a practice knowledge base including knowledge specific to the defined service area, relevant services and resources and regulatory college standards and requirements.

Required Qualifications:

Completion of master's degree in Physiotherapy from an accredited university program or bachelor's degree in Physiotherapy from an accredited university program and related experience. Active or eligible registration and practice permit with Physiotherapy Alberta College + Association.

Additional Required Qualifications:

Valid Driver's license and access to a vehicle.

Preferred Qualifications:

Able to work independently with a variety of clients in an active care setting. Excellent written and verbal skills. Proven ability to work effectively as a member of an interdisciplinary team. Previous experience in home care preferred.
