Your Opportunity:

If you are interested in working within an interprofessional team playing a key role in linking the needs of Home Care clients with appropriate health and community services, then this position will be of interest to you. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs and focus on strengths. As a Physiotherapist on the Rural East team, you are responsible for reviewing clinical referrals, accepting, screening, and assessing client needs from a physiotherapy perspective in cooperation with other health disciplines as appropriate (often part of a multidisciplinary team). You will develop, plan, implement and provide follow-up of physiotherapy services for clients in cooperation with other disciplines in the hospital and community. You are expected to establish and maintain effective communication with colleagues, medical staff, and health care practitioners, concerning documentation, client treatment plans, progress and discharge. You will participate in client/family/caregiver education in conjunction with other disciplines as required. You will operate a variety of monitoring, recording, diagnostic, therapeutic, and associated equipment. Identifies need for replacement, maintenance or repairs of supplies and equipment. You will be required to participate in the authorization of Alberta Aids to Daily Living (AADL) equipment and supplies and actively maintain current knowledge of AADL program benefits as appropriate.

Description:

As the Physiotherapist I (PT), you will provide therapeutic interventions to enable clients to achieve optimum physical function and independence in development, disease, disability, chronic conditions, illness and functional decline. You will provide intervention (i.e. prevention, triage, screening, assessment, diagnosis, goal setting, treatment, management, education, consultation, fostering resourcefulness, case management, referral and transition support to individuals, groups, populations and communities. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. The PT collaborates with patient(s) and families in determining, implementing and evaluating appropriate rehabilitation strategies. You will participate in a variety of interprofessional teams to build awareness, enrich supports and engage in quality improvement; partnering with healthcare providers, patients, families and other stakeholders to advance services for their clients. You will contribute to quality improvement and their professional community; participates in peer mentoring; and supervises students. You may provide clinical supervision to Therapy Assistants, Therapy Aides and Provisional Physiotherapists.

Required Qualifications:

Bachelor's degree in Physiotherapy from an accredited university program. Current registration with the College of Physiotherapists of Alberta.

Additional Required Qualifications:

Two years of recent home care or acute care experience in the last five years. Lower Leg Assessor. Must be proficient in computer use as role is heavily dependent on computer use. Knowledge, skill ability and willingness to work to full scope of practice. Ability to use critical thinking skills. Critical inquiry to assess, plan & evaluate.

Preferred Qualifications:

Experience working with acute clients and/or long term clients with a focus on chronic disease management. Experience in providing case management services. Experience in Lower Leg Assessment.
