Are you a patient-focused pharmacist looking to build advanced clinical expertise while making a meaningful impact in specialized ambulatory care? Alberta Health Services is seeking a skilled pharmacist to join the Southern Alberta Rare Blood and Bleeding Disorders (RBBD) Comprehensive Care Program at Foothills Medical Centre. This program delivers multidisciplinary, patient-centered care for adults with complex conditions, including congenital and acquired bleeding disorders, hemoglobinopathies such as sickle cell disease and thalassemia, and other rare hematologic disorders. In this role, you will function as an integral member of an interdisciplinary team, applying advanced clinical judgment to assess patients, develop individualized care plans, and optimize pharmacotherapy in a longitudinal clinic setting. You will build strong therapeutic relationships with patients and families through in-person and virtual care, supporting education, adherence, and shared decision-making. Collaboration with hematologists, nurses, and other healthcare professionals is essential to delivering coordinated, high-quality care. Your clinical scope includes hydroxyurea initiation and titration, iron chelation management, and comprehensive hemophilia care, including factor and non-factor therapies and pharmacokinetic-guided dosing. You will also contribute to quality improvement, research, and integration of emerging and special access therapies to advance patient care. Description:

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Required Qualifications:

Completion of Bachelor of Science in Pharmacy. Current, provisional or transferable registration with the Alberta College of Pharmacy (ACP) as a Pharmacist.

Additional Required Qualifications:

Additional Prescribing Authorization. Minimum of 1 year of recent, direct clinical experience in an ambulatory or inpatient hospital pharmacy setting within the past two years. Completion of an accredited hospital residency and/or post-professional PharmD program is highly desirable. Fluency in English, both written and verbal. Excellent communication and interpersonal skills. Ability to work both independently and collaboratively within a multidisciplinary team. Preferred Qualifications:

Completion of an accredited Canadian hospital pharmacy residency or advanced pharmacy degree (e.g., post-professional PharmD). Authorization to administer injections. Active CSHP membership. Experience in a clinical specialty, precepting learners and/or involvement in research, quality improvement, committee work is an asset. Experience or training in hematology, hemoglobinopathies, or bleeding disorders is strongly preferred. Internal medicine or heme-oncology experience is also an asset.
