Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).

Additional Required Qualifications:

Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). 3 - 5 yrs of recent direct patient care in an environment demonstrating advance practice. 3 – 5 yrs of recent experience in primary care, community health, and chronic disease management. Certified Diabetes Educator (CDE) and/or demonstrated experience in caring for and educating patients with diabetes. Authorization to administer drugs by injection. Additional Prescribing Authorization (APA). Experience in project planning, transformational change, education and effective communication skills to facilitate practice advancement. Demonstrated ability to collaborate and work with Community Pharmacies. Proficient in counseling and group facilitation. Ability to work independently and collaboratively within a multidisciplinary team. Skilled in leadership, teamwork, and relationship building. Competent in Microsoft Office, Netcare, and Electronic Medical Records. Strong written, oral, and interpersonal communication skills. Effective problem-solving and evidence-based decision-making. Excellent planning, organization, and time management. Respect and understanding of privacy legislation, including FOIP and the Health Information Act
