Your Opportunity:

The Pharmacist I working with the Home Living team provides clinical services to home living (HL) clients who may require various medication support needs. HL clients may live in a house, apartment, condominium or in a seniors residence. Home Living provides home care services for clients of all ages; however, majority of the referrals to the pharmacists are adults and seniors. These clients may require professional and support services to maintain health and independence or need palliative care support and symptom management. The HL pharmacist works collaboratively with an interprofessional team, including case managers, system case managers, nurse practitioners, Care of the Elderly Physicians within the HL program; in addition to working with other community health care providers such as physicians and community pharmacists. The pharmacist intervenes to resolve or prevent drug related problems to optimize client’s health outcomes and support independent living. The pharmacist coordinates and integrates medication therapy management services into the client care plan, provides education, follow up and documents in an electronic health record. Clinical support for the client is accomplished through client assessment, actively participating in client care conferences, liaising with members of the health care teams, developing a collaborative professional relationship with the client and their family to formulate a medication treatment and management plan. The HL pharmacist will also respond to drug information requests from staff and clients as well as participate in medication related research initiatives, quality improvement activities, and staff education. Participation in student preceptorship, teaching of pharmacy residents, staff and other members of the interdisciplinary team is required. Travel to client homes is required.

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).

Additional Required Qualifications:

Additional Prescribing Authorization. Recent pharmacy experience providing care for geriatrics, complex care, persons with multiple comorbidities and multiple medications. Demonstrates knowledge and an understanding of the current community health care programs and community-based services and resources. Demonstrated ability to use evidence-based medicine in direct patient care. Fluency in the English language (both written and verbal). Excellent communication and interpersonal skills, with demonstrated organizational and problem-solving skills. Good time management skills, including ability to prioritize, and switch priorities as needed. Self-motivated; able to work both independently and cooperatively in a team environment as demonstrated by experience working with a multidisciplinary team.

Preferred Qualifications:

Recent pharmacy experience with Continuing Care, preferably with Home and Community Care; however, Supportive (Type B Continuing Care Homes) or Facility Living (Type A Continuing Care Homes) experience is suitable. Community pharmacy experience.
