Alberta Children's Hospital is the largest public hospital for sick children in Alberta. ACH was built with family centered care in mind and prides itself on engaging patients and families on a regular basis through a family centered approach to care. ACH is a leader in pediatric and neonatal care and is a major referral center for pediatric malignancies and complex hematological treatment including pediatric bone marrow transplants. Alberta's only hospice for children (Rotary Flames House) is located next door to ACH. Responsible for the provision of pharmaceutical care to inpatients and their caregivers according to AHS, hospital and departmental standards. The Pharmacist is responsible for direct patient care services, drug distribution services (indirect patient care), drug information and education/research activities. Also includes precepting and teaching of pharmacy residents, staff, and students and provision of drug information to pharmacy, nursing, medicine and other members of the multidisciplinary team as required. Pharmacists actively participate in patient care rounds, liaise with members of the health care team and develop professional relationships with patients and families to formulate a medication treatment plan. Direct patient care responsibility will be with the hematology, oncology, transplant and immunology (HOTI) program, which provides clinical pharmacist support to both inpatients and outpatients. Drug stewardship and application of evidence based medication recommendations for the management of various cancers and their supportive treatments is an expectation. Shift length and rotation may need to be adjusted due to ongoing changing facility requirements.

As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).
Additional Required Qualifications:Advanced Prescribing Authority (APA) is required. For new practitioners or from out of province, must obtain within one (1) year. Recent direct patient care experience in pediatrics or oncology is required (at least 1 year within the last 3 years). Fluency in the English language (both written and verbal). Demonstrates computer literacy for drug information and word processing. Demonstrates ability to use evidence-based medicine in direct patient care. Demonstrates self-directed learning and continuing education. Capable of making decisions and working independently. Demonstrates good time management including ability to switch priorities and multitask. Demonstrates excellent communication and interpersonal skills, good organizational and problem-solving skills. Self-motivated; able to work both independently and cooperatively in a team environment. Demonstrated ability to work to full scope of practice. You must provide a letter of intent and indicate the reason you are applying to the ACH Pharmacy Services. If selected for an interview, be prepared to provide a Pharmaceutical Care Plan that you have recently completed.
Preferred Qualifications:>2 years' recent experience within hospital pharmacy preferred. >2 years' experience with Connect Care including Beacon, Willow and Willow Ambulatory preferred. Recent experience as a member of a multidisciplinary health care team preferred. Hospital pharmacy residency and/or advanced degree preferred. Additional experience in pediatric/oncology drug therapy strongly preferred. Project and research experience or committee work. Member of voluntary professional organization (i.e. CSHP CaPhO).
