Required Qualifications:

Completion of post-secondary degree or diploma.

Additional Required Qualifications:

Completion of post-secondary degree or diploma in SW or other relevant healthcare field. Candidates with a diploma or degree in Social Work must be registered with the Alberta College of Social Work. Candidates with another professional background subject to the Health Professions Act, must be registered with the appropriate college in Alberta. 3-5 years of experience working with families and vulnerable populations to facilitate referrals and connection to community resources and government programs is a requirement coming into this role (including experience with FSCD applications, family support programs, financial aid, housing, securing funding for equipment needs etc.). Demonstrated working knowledge of community resources and services in Alberta and Calgary for low-income families and those related to child health needs, including disability tax credits, or other child/family benefits is a requirement. Must understand the principles of Patient & Family Centered Care and demonstrate how practice of these principles improves patient and family experience. Experience working with families in high stress situations who may be experiencing distress related to their child's care. Experience and training in Connect Care is a requirement coming into this role. Exceptional communication skills and ability to work collaboratively with health care teams across the site, and with agencies and program staff in community.

Preferred Qualifications:

Knowledge of Pediatric Healthcare Services and programs that support children and families at the Alberta Children's Hospital is an asset. Experience working successfully within a multi-disciplinary team. Excellent computer skills.
