Required Qualifications:

Completion of post-secondary degree or diploma.

Additional Required Qualifications:

Completion of post-secondary degree in social work, public health or other relevant health care related field. Minimum 2 years' experience offering resource supports and referrals to community programs and services. Minimum 2 years' experience working in pediatrics. Extensive knowledge of social and community support programs focused on children with medical complexities. Knowledge of federal, provincial, regional and local resources and financial support programs for low-income families. Competent in conducting a financial assessment using strength-based approach. Efficiency working in an electronic health record system; Connect Care. Experience with Indigenous family supports and programs (e.g., Jordan's Principle, NIHB). Strong understanding of the principles of Patient & Family Centred Care and demonstrated ability to practice these principles to improve patient and family experience. Excellent current knowledge of community supports and services in Alberta and Calgary for low-income families and those related to child health needs and to tax or other child/family benefits. Strong computer skills and be capable of working within an electronic health record and show competency with other systems.

Preferred Qualifications:

Previous experience working with vulnerable families with medically complex children.
