Completion of diploma from a post-secondary program in a related field.
Additional Required Qualifications:Successful completion of a diploma (degree preferred) in a human service discipline such as Social Work. Minimum 2 years’ experience within the last 3 years working with adults with severe and persistent mental health and addiction issues. Recent community-based mental health outreach experience. Experience working with multidisciplinary teams and community agencies. Knowledge of relevant community resources and legislation. Ability to be self-directed and exercises independent decision making, and critical thinking skills. High degree of flexibility as well as the ability to work independently in an outreach capacity. Ability to work in stressful, demanding and changing conditions. Excellent interpersonal and communication skills with the ability to effectively maintain collaborative working relationships with members of the multidisciplinary team. Excellent time management and prioritization skills, with a demonstrated ability to function effectively within a dynamic environment. Demonstrated problem solving and conflict resolution skills. Actively integrates patient and family centered care principles within own practice standards. Excellent verbal and written communication skills. Basic knowledge of computers and software applications (i.e. Microsoft Office). Compliance with the AHS Hand Hygiene policies. Demonstrates behaviours consistent with the values of AHS.
Preferred Qualifications:Experience in conducting psychosocial assessments, treatment planning, crisis intervention, and medication management. Knowledge of Mental Health Legislation especially related to Community Treatment Orders. Knowledge of the ACT model. Group experience.
