Your Opportunity:

Home Care is a multi-disciplinary team with RN, LPN, SW, RT, PT, OT, and Rehab Assistants who collaborate and conduct home visits to deliver services to clients with a wide variety of conditions, including complex health needs. As an Occupational Therapist I in the Rural South Home Care Team, you will act both as a Case Manager as well as a direct care provider, utilizing your strong skills in family-centered practice, coaching, assessing, organizing and communication. The Case Management duties include ongoing assessing and identifying the overall needs of the client, in collaboration with the family and other partners, and developing a care plan based on those needs. The care plan ensures appropriate services are provided, and the appropriate internal and external resources are accessed. Case Managers work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. Please note this position is approximately 64 km SW outside of Calgary City Limits and will support primarily the Diamond Valley area.

Description:

As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.

Required Qualifications:

Completion of bachelor's degree in Occupational Therapy from an accredited university program. Registered with the Alberta College of Occupational Therapists (ACOT).

Additional Required Qualifications:

Degree from an accredited Occupational Therapy program. Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP) required. Excellent computer, interpersonal, and communication skills is required. Strong assessment, conflict resolution and time management skills is required. Proven ability to provide exceptional clinical care within a continually changing healthcare environment. Physically able to perform all assigned tasks. You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling upwards to 50 Ibs) as a regular part of the job. The successful candidate will work independently in clients’ homes, which could include single family dwellings, condominiums, and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carrying medical bags, which weigh approximately 25-30 lbs.

Preferred Qualifications:

• Familiarity with RAI, Connect Care, and Case Management principles is preferred. AADL Authorizer is an asset. • Community experience is an asset
