Your Opportunity:

As an Occupational Therapist I (OT), you will be an integral member of an inter-professional care team dedicated to supporting patients in the Palliative Home Care program. Working within the full scope of professional practice, you will provide comprehensive assessment, consultation, and treatment services to clients receiving palliative care at home. In collaboration with clients and their families, you will identify care needs, develop individualized plans, coordinate interventions, and evaluate outcomes to address both client and family goals. You will foster continuity of care through strong collaborative relationships with the inter-professional team, primary care physicians, and other programs or services involved in client care. Key Competencies include: Effective interpersonal and communication skills to build strong relationships with clients, families, and team members. Organizational and time management skills, with the ability to prioritize and manage multiple details in a timely and flexible manner. Ability to work collaboratively within an inter-professional team to ensure coordinated and holistic care. Strong decision-making and critical thinking skills to support sound clinical judgment. Self-directed practice, demonstrating independence and accountability while using professional judgment in a dynamic care environment. PLEASE NOTE: This is an ON-SITE position that requires reporting to an office site daily. There is no opportunity for remote work. *Orientation to this role is full-time for 6 consecutive weeks for applicants external to Home Care. Applicants must be able to commit to this 6 week requirement.*

Description:

As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.

Required Qualifications:

Completion of bachelor's degree in Occupational Therapy from an accredited university program. Registered with the Alberta College of Occupational Therapists (ACOT).

Additional Required Qualifications:

Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) required. A minimum of 1 year of current, relevant OT experience is required. AADL Authorizer Proficiency with Connect Care, Netcare, and Microsoft Office Suite to document, communicate, and manage client information effectively A valid driver’s license and a reliable vehicle is required The Occupational Therapist I may be required to maintain a standing position and perform up to a medium level of work (20–50 lbs. handled occasionally), including bending and rotation related to client transfers or therapeutic interventions Ability to perform all duties in accordance with related legislation.

Preferred Qualifications:

Preference will be given to candidates who have relevant OT experience in a community setting. Current knowledge and skill related to palliative care OT is preferred.
