Your Opportunity:

Reporting to the Care Manager for Specialized Geriatric Services, this position performs a specialist role responsible for assessment, consultation, and short-term interventions where appropriate. The incumbent will also incorporate a blend of transdisciplinary and interdisciplinary approach to care in collaboration with other disciplines on the team. The Occupational Therapist I (OT) will conduct comprehensive geriatric assessments or components of assessment within the team, document findings, provide interventions and delegations to support personnel, as well as participate in rounds if required. The incumbent will purposefully connect with internal and external partners for care planning e.g. liaise with care partners including Integrated Home Care, family physicians and other specialty clinics along the continuum as required. The incumbent will demonstrate informal leadership skills through effective communication, ethical decision making and commitment to achieving established goals and objectives, ensuring outcome measurements and standards are enforced.

Description:

As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.

Required Qualifications:

Completion of bachelor's degree in Occupational Therapy from an accredited university program. Registered with the Alberta College of Occupational Therapists (ACOT).

Additional Required Qualifications:

Demonstrated ability to problem solve and work autonomously. Demonstrated ability to function effectively within a patient and family centered team of health care professionals and support staff. Experience in providing delegation/assignment and supervision to support personnel who are carrying out those assigned tasks and/or service components. Demonstrated and clear pattern of professional and personal development. Demonstrated excellent verbal and written communication skills. Demonstrated excellent conflict resolution skills. Demonstrated ability to effect change and create innovative solutions for complex and diverse issues. Demonstrated excellent organizational and time management skills. Advanced computer skills including the use of Microsoft Office required. Proven initiative, critical thinking and decision-making skills physically able to perform all assigned tasks including transfers and lifting.

Preferred Qualifications:

3-5 years recent community experience. Experience in a transdisciplinary community practice setting Experience with holistic community-based OT assessments. Membership with the Canadian Association of Occupational Therapy (CAOT). Ambulatory, community or outpatient experience in seniors’ health or medically complex populations. Leadership experience in mentorship (of other staff and/or students) as well as teaching. Strong understanding of the needs along the continuum of care for seniors.
