Successful completion of an approved Nurse Practitioner program and a minimum of master's degree. Registration with CRNA on the Nurse Practitioner Registrar including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Minimum of 5 years of clinical experience as a Nurse Practitioner. Minimum 1 year of clinical experience managing complex clients with dementia (within the last 3 years). Minimum 1 year of Seniors Health experience. Advanced clinical & assessment skills. Connect Care trained. Competent with MS Office Suite, & TEAMs. Proven leadership experience. Possession of a valid driver's license and access to a motor vehicle in good repair for work-related travel. Able to work independently in different environments (weather conditions) and must be physically capable of carrying approx. 25-30lbs.
Preferred Qualifications:Experience in at least 2 care sectors, including Seniors Health. CNA Certification in Geriatrics, or willingness to obtain. Case Management Education. Experience in program planning, education of adults, interdisciplinary practice, and research activities. Excellent interpersonal & communication skills. Ability to work independently & to be flexible to adapt to changing clinical situations. Works collaboratively with all members of the health care team, including patients and their families.
