Join Assisted Living Alberta’s Facility and Supportive Living team as a Nurse Practitioner and work alongside an experienced team of NPs dedicated to providing high-quality, person-centered community care. In this role, you’ll help clients feel safe & supported in the place they call home. In this role, you’ll provide person centered care to clients in congregate living settings, focusing on health promotion, well-being, independence, and maintaining functional abilities. You’ll use your advanced assessment skills and holistic understanding of aging and chronic disease to stabilize health concerns, reduce emergency department visits, and prevent avoidable hospital admissions. Collaboration is essential. You’ll work closely with interdisciplinary teams, physicians, case managers, and care operators to deliver seamless, coordinated care tailored to each client’s needs. You’ll also support clients and families as they navigate complex health decisions, including chronic disease management and palliative or end of life care when needed. This position offers a blend of office-based work and community visits across established care networks in the Edmonton area, serving clients who may be homebound, unattached to primary care, ineligible for specialized programs, or living with complex medical, cognitive, or social needs. Description:

As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Required Qualifications:

Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

At 3 least years of recent nursing experience across a combination of practice areas, which may include: acute/sub acute care, rehabilitative care, community health, emergency, ICU, continuing care, home care, or related settings. Strong communication and assessment skills. Evidence of expertise aligned with the Nurse Practitioner graduate stream, such as Family/All Ages or Adult. At the time of interview, candidates may be required to complete a hands on computer assessment to demonstrate proficiency. This includes showing sufficient typing speed to meet workload demands and the ability to confidently navigate various computer programs such as Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook, & internet-based applications.

Preferred Qualifications:

Preferred qualifications include CNA Certification in a national nursing specialty (e.g., Gerontological Nursing, Critical Care Nursing, Hospice and Palliative Care Nursing). Connect Care experience is an asset, as is previous experience in Palliative Care.
