Assisted Living Alberta Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. The Palliative and End-of-Life Care Program is recruiting an energetic and adaptable Nurse Practitioner (NP) who can manage a specialty clinical workload autonomously. The ideal candidate would be a motivated, self-starting, and proactive individual, committed to lifelong learning and excellent service delivery in the PEOL program. Are you someone who is passionate about PEOL, who has a drive to continually grow and achieve team and system- oriented goals? This is an NP palliative consultant role on the Palliative Community Consult Team (PCCT). You will work alongside a specialized team of palliative physicians, one other NP, and palliative nurse consultants to address referrals related to PEOL symptoms that are difficult to manage, complex family issues related to patients’ illnesses, disease comprehension discussions, complex GCD and ACP discussions, and Type C Continuing Care Home (hospice) assessments. You will work with system collaborators by assessing patients in their private residence, those residing in Type A/B/C Continuing Care Homes, and those as in-patients in Zone suburban/rural community hospitals. This work involves working with clinical partners in care: primary care providers, the Cross Cancer Institute, home care teams, Continuing Care attending teams, and acute care teams. You will work in an office setting, as well, will travel within established geographical networks of care in the Edmonton Zone. Your day-to-day work changes based on the location of the patients.

As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Required Qualifications:

Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Consistent, proven, & demonstrated experience with managing & working on multiple competing priorities, requiring fast action & re-prioritization. Works well under pressure. Works to CRNA full scope of advanced clinical practice: accesses & utilizes resources available & integrates them into scope related clinical expertise to make informed decisions about patient medical care in alignment with patients’ GCD, individual preferences & wishes. In alignment with evidence-informed practice: provide advanced health assessments & diagnosis, order labs & diagnostic tests, prescribe pharmacotherapy; provide treatment, treatment plans, & advanced interventions, monitor patient outcomes, & consult &/or refer to specialty services. Communication (written, verbal, non-verbal): highly skilled and effective to write accurate, clear, concise, timely, & relevant medical consult notes; to communicate with a large scope of system collaborators, & with patients & their caregivers during times of distress and urgency. 5 years of recent (within the last six months) NP experience in one or more practice settings. Evidence of expertise in your graduate degree, NP stream (e.g., Family/All Ages). Ability to travel throughout the AHS Edmonton Zone year-round.

Preferred Qualifications:

CNA Certification in National Nursing Specialty (i.e., Gerontological, Critical Care, Nursing, Hospice, Palliative Care Nursing, etc.). Connect Care experience. Palliative and end-of-life care experience.
