Are you a Nurse Practitioner looking for a dynamic and meaningful opportunity to make a difference in the lives of older adults? As the most responsible health practitioner (MRPH), this role offers the chance to provide direct client care to complex clients who are homebound, medically frail, and experiencing significant barriers to accessing primary care in traditional environments. You will work in close collaboration with Seniors Home-Based Primary Care (SHBPC) physicians, who serve as the Most Responsible Practitioner, ensuring coordinated and comprehensive care. In addition to supporting SHBPC clients, you will also provide direct client care to clients in the In-Home Restorative Care program (IHRCP). Clients in the IHRCP have recently experienced a decline in their functional ability. Whether due to a health setback or following a medical event, with the goal of supporting clients to regain their strength, mobility, and functional independence. You will work in close collaboration with the client’s primary care physician. The position is embedded within a blended interdisciplinary and transdisciplinary model of care, offering a rich environment for collaboration and innovation. Description:
As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Active registration with the College of Registered Nurses of Alberta (CRNA) as a Nurse Practitioner in the appropriate stream (e.g., Family/All Ages, Adult). Completion of a Master’s degree in Nursing is required. This position requires a vehicle for business use. Possession of a valid driver’s license and access to a motor vehicle in good condition for work-related travel is required. Knowledge of and ability to support change management. Advanced Microsoft office skills (Power Point, Excel) with proficiency in MS Outlook and MS Teams. You will be responsible to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example, A push/pull of up to 50lbs for compression therapy on a regular basis is an expectation of this job. Learning resources and training are provided to safely perform work. Minimum of three years’ experience with older adults in the community within the last five years. Experience in research and best practice implementation. Recent experience working as an NP with the older adult population. Proven experience with teaching, supervision, coaching and mentorship. Ability to collaborate with stakeholders within and outside of AHS. Proven initiative, autonomy, conflict resolution & critical thinking skills.
Preferred Qualifications:Demonstrated excellent written & verbal skills with continuing education in the area of the older adult. Demonstrated self-directedness in organizing work prioritize & flexibility in meeting the unexpected spontaneous needs of a clinical area. Demonstrated ability to function effectively & collaboratively within a client and family centered care team of health care professionals and support staff. CNA certification in Gerontology.
