Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Nurse Practitioner to join our Facility & Supportive Living team to provide care and support to adults and seniors residing in congregate living settings. Supportive Living is a non-institutional alternative for those who are no longer able to live in their own home, which focuses on health promotion, well-being, independence and maintaining functional abilities. As a Nurse Practitioner in Continuing Care, you will have the opportunity to work in an office setting as well as travel within established geographical networks of care in the Edmonton Zone to address clients who: are homebound; unattached to primary care physicians; do not meet the criteria for special programs and services, are complex and vulnerable geriatric clients and for those whose social, cognitive and medical complexity provide challenges for ongoing management in the community. The successful candidate will assume a key role in the management of clients in the community, building on primary health care concepts and case management principles, thereby diminishing emergency room visits, acute care admissions and re-admissions, mitigating adverse event management, assisting with medication reconciliation and assisting in the care of complex client scenarios such as palliative and end of life care. Within this advanced model of care, you will work as a partnership with a common goal of clients achieving and maintaining their health, well-being, and independence.

Description:

As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Required Qualifications:

Successful completion of an approved Nurse Practitioner program and a minimum of master's degree. Registration with CRNA on the Nurse Practitioner Registrar including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Communication and assessment skills. 3-5 years of recent nursing experience in a combination of areas including but not limited to: acute/sub-acute, rehabilitative, community emergency room, ICU, continuing care, home care, etc. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet. Evidence of area of expertise in their graduate degree Nurse Practitioner stream (Family/All Ages, Adult, Child/Child (Neonatal Specialty).

Preferred Qualifications:

CNA Certification in National Nursing Specialty (i.e., Gerontological, Critical Care, Nursing, Hospice, Palliative Care Nursing, etc.). Meditech and Connect Care experience. Palliative Care experience.
