Minimum of 3 years' of recent clinical experience as a NP, with at least 1 year experience working with older adult population. CNA certification in geriatrics. Evidence of knowledge of Continuing Care service delivery options, Designated Living Option (DLO) assessment/placement criteria, and community supports and agencies. Ability to adapt skills to a virtual working environment. Proven experience of successful teaching. Proven experience in program development. Demonstrated ability to function collaboratively and effectively as an inter-professional team member. Exhibit strong time management and prioritization skills, with a demonstrated ability to function effectively both independently and within a dynamic team environment. Successful completion of an approved Nurse Practitioner program at a minimum of a Master of Nursing level or equivalent. Active or eligible for registration as a Nurse Practitioner with the College and Association of Registered Nurses of Alberta with a practice permit indicating appropriate stream of practice. Experience within Continuing Care streams of Home Care, Designated Supportive Living, and/or Long Term Care. Willingness to complete additional education and/or certifications specific to role.
Additional Required Qualifications:
Demonstrated ability to provide support and advice to Continuing Care teams including but not limited to care planning, assessment, care interventions, and development of programs for CC clients with complex cognitive/mental health/behavioral concerns, palliative care needs, complexities of chronic illness, co-morbidities and frailty, for example. Demonstrated ability to actively integrate Client and Family-Centered-Care principles into practice. Excellent communication and interpersonal skills in order to work collaboratively with members of the inter-professional team. Proven experience in critical thinking, conflict resolution and problem solving skills. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Minimum of three (3) years' experience.
