Your Opportunity:

The Nurse Clinician - Diabetes Nurse Educator functions as a member of the multidisciplinary team to deliver in-depth diabetes education and provide clinical management to clients and families in AKC-S. The Diabetes Nurse Educator will provide care to adults in all stages of chronic kidney disease (CKD) and disease complexity. You will work closely with all members of the health care team, such as the Nephrologist, Nurse Case Manager, Pharmacist, Dietician and Social Work to ensure that all aspects of renal and diabetes care have been addressed and integrated into the plan of care. Where appropriate and consistent with the patients' wishes, education will promote living donor transplantation, home dialysis, and non-dialysis conservative care. You will provide leadership in assessment, planning, implementation, and evaluation of care for the purposes of promoting, maintaining, or restoring health, and managing multiple comorbidities. The responsibilities and functions of the Diabetic Nurse Educator are performed at full scope of practice. Practices are in accordance with employer policy and professional standards, including Standards of Diabetes Education in Canada and Clinical Practice Guidelines. You will work out of Sheldon M Chumir Health Centre but will occasionally be required to travel and perform duties at multiple Alberta Kidney Care-South sites within Calgary along with virtual care provided to sites outside of Calgary.

Description:

As a Nurse Clinician (NC), you will be responsible for the coordination of an interprofessional team, in the provision of quality patient (client, resident) care in order to meet the operational priorities and requirements within a clinical setting. You will use an integrative leadership approach in assisting, planning, implementing and evaluating care needs, while assuming responsibility for staff assignments, patient flow and clinical resources. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Baccalaureate Degree in Nursing from an accredited university. Experience working with clients with diabetes within the last two years. Demonstrated computer and electronic health record documentation skills. Experience with patient education and application of the principles of adult learning. Expertise in interviewing, counseling, teaching and group dynamics. Ability to work independently but collaboratively with clients, families and multidisciplinary team members. Experience working with family physicians/specialist physicians. Demonstrates initiative, accountability, good time management, decision making ability and strong organizational skills. Certified Diabetes Educator (CDE) designation. Minimum of 3 years of nephrology disease management experience in the last 5 years.

Preferred Qualifications:

Member of Diabetes Education Section (DES) of Canadian Diabetes Association (CDA) encouraged. CNeph(C) Certification. Nurse Practitioner certification. Experience with insulin pump therapy. Case Management experience within the last 5 years.
