Description:

The Manager, Transition Services, provides strategic and operational leadership to clinical teams supporting patient flow and care transitions across the Central Zone. This role leads multidisciplinary teams who coordinate safe, person‑centered transitions from emergency and acute care settings into community programs—ensuring timely access to appropriate services, supporting continuity of care, and improving overall system flow. Reporting to the Transition Services Director, the Manager champions innovation, quality improvement, and integration across Acute Care, Primary Care, Community programs, and Assisted Living Alberta. The role oversees Transition Coordinators and specialized assessment teams, supports process redesign, monitors performance indicators, and fosters collaborative relationships with site leadership, physicians, partner programs, and community agencies. This position plays a key role in operational planning, staff development, workload management, and change leadership. The Manager ensures services align with relevant standards, legislation, and provincial policies while cultivating a culture of safety, accountability, and continuous learning. The role also supports resolution of complex client and family concerns, contributes to program planning and evaluation, and maintains effective communication across diverse stakeholder groups.
