Your Opportunity:
Reporting to Area Manager of the Transition Services, the Care Manager performs a key leadership role and is responsible and accountable for providing the direction, team leadership and organization over the activities of the Peter Lougheed Transition Services in support of the vision, mission and business plan of Alberta Health Services – Please note this position will be transferred to ALA effective Sept 01 2025. The Care Manager will be supporting the Transition Services team across emergency and inpatient care. The Care Manager should be aware of Transition Services processes and be familiar with the TS role. This involves supporting the team through extensive assessment, facilitation and negotiation to develop the plan of discharge. The Care Manager must have excellent knowledge of community agencies and resources to facilitate the linkage to appropriate services.

The Care Manager is responsible for establishing and maintaining an effective framework for the efficient day to day operations by making unit–level decisions, overseeing the implementation of and ongoing measurement of patient outcomes and developing and maintaining strong working relationship with the Transition Services team including staff, patients and families. This also includes working collaboratively with key stakeholders within the programs and services at the Peter Lougheed Hospital. The Care Manager is accountable for the hiring, development, and management of professional and ancillary staff. In addition, the Care Manager facilitates the implementation of changes to the structure and processes within the program, to achieve improved outcomes related to patient satisfaction, quality care, and cost containment. You will provide leadership through effective communication, ethical decision making, and commitment to achieving established goals and objectives, ensuring outcome measurements and standards are enforced.

Graduate of a recognized health program with a Baccalaureate degree or equivalent required. Current College of Registered Nurses of Alberta (CRNA) or health profession registration required. Excellent interpersonal, communication and leadership skills. Self-directed and highly motivated. Analytical, organizational, planning, teaching and decision-making skills. Positive, creative perspective, high energy to support site, zone and provincial initiatives. Experience with staff management and performance evaluation.
Additional Required Qualifications:Demonstrated ability and desire to balance needs of patients along with system pressures. Experience and understanding of health care systems, change management, quality improvement and clinical practice guidelines. Leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching and teaching and the ability to inspire and build confidence in others. Proven ability to foster partnerships and to achieve organizational goals within a diverse organization. Ability to collaborate and negotiate with external and internal multidisciplinary teams. Demonstrated and clear pattern of professional and personal development. Strong computer literacy skills.
Preferred Qualifications:Minimum two-three years formal leadership experience with demonstrated clinical competence and knowledge of Continuing Care. Experience managing a unionized workforce. Master's Degree in a relevant discipline. Front Line TS experience would be preferred, along with knowledge of patient flow in emergency, inpatient and ambulatory settings.
