Rocky Mountain House has an exciting opportunity for a Licensed Practical Nurse (LPN) in Home Care! Located within the Rocky Mountain House Health Centre, you will work with a dynamic, supportive team and will have the opportunity to make a difference in the lives of people in your community. Reporting to the Care Manager, the LPN is responsible for directing the care of clients under the home care program. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning and facilitate client and family teaching. Through the development of the care plan, you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. Your experience in providing wound care and knowledge of the wound care principles and products, as well as palliative and home parenteral therapy will be essential to making you successful in this position. Rocky Mountain House is located 77 km west of Red Deer in west – central Alberta, on the North Saskatchewan River. This multicultural community has that small town feel with big city amenities. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:As Required.
Preferred Qualifications:Minimum of 2 years Home Care experience within the last 5 years of nursing preferred, although, skills and experience relevant to nursing practice in community sector will be considered. Current use of Connect Care. Experience with HPT, wound care, client teaching, palliative support. Excellent interpersonal, communication, time management and ability to work in a collaborative team demonstrating adaptability and flexibility.
