Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:The LPN is responsible for her/his nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). The LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. There is also an expectation to be a mobile worker. This role requires travel to care for clients in their homes or in the community. Possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes is a requirement of this position. You will be responsible to carry and move equipment, use, and operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing, and pulling) as a regular part of the job. Learning resources and training are provided to safely perform work. Completion of an accredited Licensed Practical Nurse education program and registration with the College of Licensed Practical Nurses of Alberta (CLPNA), the Case manager LPN will work within their scope of practice when conducting assessments, determining service needs, negotiating service options, making service recommendations and referrals and as a result continues to align with the current Standard provincial Job Description for the LPN role. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) from the Heart and Stroke Foundation within the past year Minimum of two years current nursing experience with one year in a community-based setting
Preferred Qualifications:Completion of a post-basic Medication Administration Course for LPN’s Recent work experience in a community-based setting 3 years Acute Care experience preferred. 2 years Home Care experience preferred. Experience in case management practice and completion of McMaster AHS Case Management Education preferred.
