Completion of bachelor's degree in Health Promotion, Health Sciences, Health Education or a related field.
Additional Required Qualifications:Current CPR (BCLS). Lay Leader Certification in the Stanford Chronic Disease Self-Management Program. Experience or certification in self-management support, patient education, behavior change, and adult education. Minimum of three years’ experience in community or primary care and/or experience with self-management support, chronic disease management and prevention.
Preferred Qualifications:Additional training and skills in patient-centered care, self-management support and behavior change principles. Experience with applying the Expanded Chronic Disease Care Model . Evidence of ongoing personal/professional development and learning.
