Completion of bachelor's degree in a health related field.
Additional Required Qualifications:Minimum of three years' experience in community health based adult education and working with persons with chronic illnesses. Experience educating health professionals and peers. Experience working with Vulnerable populations.
Preferred Qualifications:Master’s level education or equivalent experience desired. Minimum of three years' experience and leadership in program planning, coordination, delivery and evaluation desired. Evidence of ongoing professional development in the fields of behavior change, HealthChange ® Methodology, self-management and adult education. Experience with primary care, specialty care and work experience in varied community settings, including working with marginalized populations, is an asset.
