Completion of bachelor's degree in a health related field.
Additional Required Qualifications:Current registration with appropriate Provincial/National Professional Associations as required by legislation. CPR Basic Level. Valid Alberta Driver’s License – Class V. Credentials in Behaviour Change principles or management, such as formal education or certifications. Evidence of current and ongoing professional development in Behavior Change management and/or chronic disease self-management. Demonstrated success in performing environmental scans, identifying, developing, implementing and evaluating behaviour change programs and interventions for clients and providers. Applied experience with models of self-management, patient-centered care, and chronic disease management.
Preferred Qualifications:Master’s Degree is an asset. Additional training in patient-centered care, self-management support, and the Expanded Chronic Care model are considered assets. A minimum of three years' experience in community health is preferred.
