Required Qualifications:

Active or eligible for registration and practice permit with the applicable college.

Additional Required Qualifications:

Baccalaureate Degree in a health discipline, education, and/or leadership/administration. Minimum of 5 years of recent Home Living experience. Experience with development, implementation, facilitation and evaluation of education materials and programming is required, within the Home Living portfolio. Basic Cardiac Life Support – Healthcare Provider (BCLS-HCP) is required. Ability to evaluate clinical performance, assess staff clinical needs and plan appropriate education strategies. Demonstrated leadership and facilitation skills. Strong collaborative and interpersonal skills as well as excellence in written and verbal communication. Coaches others and promotes teamwork and partnerships to attain optimal client outcomes. Recognized for clinical expertise in Home Living Program areas. Demonstrates creativity, flexibility and initiative. Displays highly developed organizational skills, critical thinking skills and problem-solving ability. The successful candidate will work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry supply bags, which weigh approximately 25-30 lbs. At the time of interview, you will be required to demonstrate your computer knowledge and proficiency by completing a skills test. Must have working knowledge of computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet

Preferred Qualifications:

Post graduate studies or certification in adult education, gerontology, chronic disease management and/or palliative care would be an asset. Masters degree in a health-related field is an asset. Participation in research activities is an asset. Management experience is an asset. Project management is an asset.
