Active or eligible for registration and practice permit with the applicable college.
Additional Required Qualifications:Bachelor’s degree in one of the following disciplines: Occupational Therapy, Physical Therapy, Dietician, Speech Language Pathology, Social Work, etc. A minimum of 3 years of recent health care experience is required, as well as a broad knowledge of programs offered by the Continuing Care portfolio. Basic Cardiac Life Support. Demonstrated effective communication, interpersonal and organizational skills. Proven ability to establish, foster and maintain effective working partnerships (both internally and externally) to achieve organizational goals within assigned work. Established leadership and facilitation skills. Demonstrated creativity, flexibility, and initiative. Experienced in using data for quality improvement. Technical skills in computer software and programs such as Microsoft Office, Word, PowerPoint, Excel, Visio, Zoom and MS Teams to deliver virtual education. Frequent physical demands include lifting, pulling, and pushing, and moving equipment; sitting and standing for extended periods. Able to work in multiple workplace settings and carry supply bags approx. 35-50 lbs. Valid driver’s license, applicant must have access to a motor vehicle in good repair. Applicants could be asked to work extended hours and shifts.
Preferred Qualifications:5 years’ experience working in Continuing Care as a front-line clinician or educator. Master’s Degree health sciences, business, or education. Experience working in the following: Meditech, RAI, Case Management and Wound Care. Proficiency as an educator, facilitator, or trainer. Certification in project management or experience with allied health professionals in a project management capacity.
