Diploma or Degree in Nursing (RN, LPN). Minimum four years' relevant experience. Current license or active registration from appropriate professional body is required. Equivalencies of education and experience may be considered.
Additional Required Qualifications:
Experience in Seniors Health/Continuing Care sector (related experience may be considered). Excellent communication and report writing skills. Must use critical thinking and creative problem solving when approaching tasks. Ability to interpret and analyze data, and conduct risk assessments. Proficient in Microsoft Office, Word, Excel, PowerPoint, Outlook. Able to work independently, with minimal guidance. Strong organizational skills. Ability to cope with demanding timelines - works well under pressure. Strong commitment to work collaboratively in a team environment to achieve common goals. Ability to work with multiple stakeholders.
Preferred Qualifications:
Proven project management and quality improvement experience an asset. Case Management education. Experience with RAI - HC and RAI 2.0. Interpreting client care records/ hospital documentation. Interpreting Continuing Care Health Service Standards (CCHSS). Knowledge of Alberta Improvement Way (AIW) methodology.
