Required Qualifications:

Active or eligible for registration with applicable professional association or college.

Additional Required Qualifications:

Strong knowledge of applicable legislation (Personal Directives Act, Adult Guardianship and Trusteeship Act, etc.), the health care system, and community resources are required as this position is a resource for the interdisciplinary team. Specific skills in Case Management, group facilitation, adult education, and adjustment/supportive counseling are assets. The client group served is diverse; clients may be adults of any age. Comfort and experience with a diverse population including those with acquired brain injuries, and mental health challenges and dementia and the behaviors that maybe associated to this population is required. Services to clients may be provided in the hospital, home setting, congregate living setting, shelters or care home setting.

Preferred Qualifications:

Very strong communication skills and the ability to function both independently and as a contributing member of the interdisciplinary skills both within AHS and external agencies are key to this role, as is the ability to balance multiple demands in dynamic environments.
