Bachelor’s degree in health related field required. An equivalent combination of experience and education will be considered. A minimum of 5 years of clinical practice or health related experience – community care preferred.
Additional Required Qualifications:
Annual membership in good standing with an accredited professional association is required and must be maintained throughout the duration of employment.
Preferred Qualifications:
Master’s degree and previous management experience in a health related field is preferred. Computer literacy in Microsoft Outlook, Word, ESP, e-People and electronic meditech/connect care documentation is an asset. Knowledge of the principles of adult learning is an asset. McMaster Case Management training and understanding of the AHS Continuing Care Case Management Competency Framework.
